Release of Information
In order for the Office of Disability Services to ask for additional information from other professionals (such as the student's physician, high school counselor, or psychologist), the student must sign a Release of Information Form.
The student should fill out the form and send it in with his/her documentation of a disability. If the student is less than 18 years old, a parent must sign the form.
