- All Supervisors will receive information from the Office of Human Resources regarding the annual performance evaluation process in September of each year.
- Non-probationary, classified employees should be evaluated using the "Performance Planning and Evaluation" forms at the back of the Employee Work Profile.
- Employees should be encouraged to do a "self-assessment"
- The evaluation session should be conducted using the Performance Evaluation Session guidelines from the earlier section.
- Evaluations must be submitted to HR by the deadline and will remain in the employee's personnel file.
Extraordinary Performance:
Document using the "Acknowledgment of Extraordinary Contribution" form.
Must attach to the overall performance evaluation to substantiate the high rating.
Substandard Performance:
- Document using the "Notice Of Improvement Needed/Substandard Performance" form.
- Must give at least one "Notice of Improvement Needed/Substandard Performance" notice during the performance cycle to the employee if you decide to give a "Below Contributor" rating on overall evaluation.
- A development plan must be created with the employee within 10 workdays of the evaluation meeting. The performance re-evaluation plan sets forth performance measures for the following 3 months.
- The employee must be re-evaluated 2 weeks prior to the end of the 3 month re-evaluation period.
- If the employee receives a re-evaluation rating of "Below Contributor", consult with HR concerning the options (transfer, demotion, or termination).
