Request for Special Payment for Teaching and Administrative Faculty

The Special Pay form applies to full- time teaching faculty and adminstrative faculty who are paid for job assignments in addition to their regular full-time employment.

The Special Pay Form is completed by the supervisor of the second job assignment and should include the following items:

  • The employee’s signature,
  • The employee’s primary supervisor’s signature, and
  • Fund, Organization, Account, and Program (FOAP).

The second job supervisor forwards the form to the Office of Human Resources for signature approval and data entry.  The Office of Human Resources will forward the completed form to the Payroll Office.  The special payment request form must be received in Human Resources by the 1st or 15th of the month to ensure timely payment in the next paycheck.

Special payment forms will be maintained in the employee's primary personnel file.
