Instructions for Completing the Administrative and Professional Faculty Work Planning and Appraisal Form

The Administrative and Professional Faculty Work Planning and Performance Appraisal Form consists of five parts.

Part I--Job Profile

  • Employee information
  • Supervisor information
  • Goal statement
  • Major job responsibilities
  • Competencies/knowledge, skills, abilities (KSAs) needed for job
  • Education, experience, licensure, certification for entry in job


Part II--Work Plan and Appraisal for Each Responsibility for Work Year

  • Responsibility
    1. Representative duties, tasks, projects, and special assignments for responsibility
    2. Professional development plan
  • Self-assessment by employee
    1. Self rating for results
    2. Self rating for process (how you did it)
  • Assessment by supervisor
    1. Comments by supervisor
    2. Supervisor rating for results
    3. Supervisor rating for process (how you did it)
    4. Development comments (by supervisor, based on appraisal)

Part III--Other Annual University Requirements at Start of Work Year and Signatures for Work Plan Components and Other Annual Requirements

Performance Planning

The following portions of the Administrative and Professional Faculty Performance Planning and Appraisal Form should be completed by June 25 of each year. For new employees, the Individual Work Plan should be established by the supervisor and the employee no later than 30 days from the date of employment.

  • Part I --Job Profile
    1. Supervisor completes the employee and supervisor information
    2. Supervisor includes Goal Statement
    3. Supervisor and employee establish up to three major responsibilities for employee
    4. Supervisor lists Competencies/KSA’s and Education, Experience, etc. needed for the job
  • Part II--Work Plan and Appraisal for Each Responsibility for Work Year
    1. Each responsibility is automatically filled in from Part I (by pressing “print preview”)
    2. Supervisor and employee develop representative duties, task, projects, and special assignments for each responsibility
    3. Supervisor, in consultation with employee, sets the professional development plan for each responsibility
  • Part III—Other Annual University Requirements at Start of Work Year
    1. Supervisor obtains reviewer’s signature on Part III before presenting to employee to sign
    2. Supervisor meets with employee to discuss the Individual Work Plan
    3. Supervisor discusses the Additional Institutional Priority Appraisal Elements (in Part V) with employee
    4. Employee completes Mandatory Checklist and signs form
    5. Supervisor gives employee copy of form and keeps the original (as an electronic copy) in supervisor’s files

Mid-Cycle Review

This portion of the form should be completed by January 25 of each year in order to verify that a mid-cycle review was done for each employee.

  • Part IV—Mid-Cycle Developmental Review
    1. Supervisor meets with employee and discusses developmental review
    2. Supervisor checks box and signs to indicate review has been done
    3. Employee checks box and signs to indicate review has been done
    4. Supervisor maintains signed form

Annual Performance Appraisal

The following portions of the form should be completed by June 1 of each year to complete the appraisal process:

--Employee’s Role and Responsibility

Employee completes appropriate portions of Part II and Part V and sends back to supervisor by dates that were established by supervisor at the mid-cycle review.

  • Part II
    1. Employee receives an electronic copy of the form from their supervisor
    2. Employee completes a self assessment on each responsibility
    3. Employee rates each responsibility on results and process
    4. Employee can insert a short description or attach a supporting document
  • Part V
    1. Employee can make comments on any of the Additional Institutional Priority Appraisal Elements
    2. Employee returns electronic copy of form to supervisor
    3. After supervisor completes the appraisal form, employee meets with supervisor to discuss appraisal

--Supervisor’s Role and Responsibility

Supervisor sends electronic copy of form to employee. Supervisor will receive and review the employee’s self assessment and follow up with employee if there are any questions. The supervisor will complete Part II and Part V and send to reviewer for signature before meeting with and presenting the final copy to the employee.

  • Part II
    1. Supervisor makes an assessment of each responsibility and will indicate a rating for results and process
    2. Supervisor comments on development plan for each responsibility
  • Part V
    1. Supervisor rates and makes comment on Additional Institutional Priority Appraisal Elements
    2. Supervisor completes overall appraisal and assigns an overall rating
    3. Supervisor signs and sends the appraisal to the reviewer, and obtains reviewer’s signature before meeting with employee
    4. Supervisor meets with employee to discuss the annual appraisal
    5. Supervisor obtains employee’s signature, gives a copy to the employee and keeps a copy in supervisor’s file, and sends entire form to Human Resources for employee’s file

--Reviewer’s Role and Responsibility

The reviewer should review the supervisor’s annual appraisal of employee performance and take into consideration the department work plan, individual work plan, and the employee’s self-assessment. The reviewer is also responsible for equity across his/her division.

  • Part V
    1. Reviewer signs Part V and makes comments, if desired
    2. Reviewer returns form to supervisor
