Entering and Updating your Emergency Contact Information

After you have entered new emergency contact information, please print the form, print your name clearly on the page, and then sign your name and return it to the Office of Human Resources.

Adding more than one phone number for an Emergency Contact

You may add more than one phone number for an Emergency Contact person by clicking on "new contact" after submitting the initial information.  In this second entry, fill in the person's name only and then the additional phone number.  In the extension box, you may indicate the type of telephone (ie. "cell", "work").

Finance SSB Users

You do not need to create a new password when using Banner Employee Self Service.  Continue using the same password you use for Finance SSB.
