Instructions on how to Complete the Request for Counseling Form
- Complete both pages of the "Request for Counseling" form, including the demographic section.
- Print the request form, sign, and date it.
- Print, sign, and date the "Client Rights and Responsibilites" form.
- Submit both forms by mail or fax or by dropping them off at the office.
- You will be contacted by a counselor to schedule an appointment.
Your request for counseling form will not be accepted by either RRSBDC office if:
- Both pages are not completed
- The request form is not signed or dated
