Instructions on how to Complete the Request for Counseling Form
- Complete both pages of the Request for Counseling Form, this initial information is crucial to the counseling process and for maintaining our records according to SBA guidelines. The demographic information on this form is confidential and is used only for statistical purposes.
- Print out the completed Request for Counseling Form,sign and date it, and return it to the appropriate RRSBDC Office. They can be mailed, faxed, or hand delivered.
- Once your request form as been received by the appropriate RRSBDC office and logged in, a counselor will contact you.
- Our counseling sessions are free, because we are sponsored by the SBA and the University of Mary Washington.
Your request for counseling form will not be accepted by either RRSBDC offices if:
- Both pages of the form are not completed
- The request form is not signed or dated
So, please make sure you look over your request form before you return it to us.
