| Request for Foundation Funds | Faculty and staff should use this form when requesting payment of invoices, individuals, or reimbursement from Foundation accounts. The form must be signed by the faculty/staff member submitting the form and when emailed as instructed, must include all documentation (invoice, etc.) in one file. A scan/PDF is recommended. Please read full instructions. |
| W-9 Form | When paying individuals with Foundation funds, this form must be completed and submitted by the individual receiving payment before payment can be issued. |
| Sales Tax Exemption | When purchasing items with Foundation funds, submit this form to vendors to ensure that sales tax is not charged. |
